Funded by the Vermont Blueprint for Health initiative, the Lamoille Health Community Health Team supplements the primary care services provided by our Family Medicine and Pediatric providers by connecting patients with the social and financial services that make healthy living possible.

Our team has a deep knowledge of the community services available to patients, as well as contacts at each, which helps ensure patients get quick access when they need it.

Consisting of care coordinators and case managers who are Registered Nurses, Registered Dietitians, Certified Diabetes Educators, and Social Workers, the Team often works alongside a patient’s primary care physician (PCP), providing education and support that help patients make healthy lifestyle changes.

  • Find affordable prescriptions
  • Assist in managing chronic medical conditions
  • Deliver personalized healthy living guidance
  • Enroll in Healthy Living programs that cover:
    • Diabetes Prevention
    • Diabetes Management
    • Chronic Medical Conditions
    • Chronic Pain
    • Smoking Cessation

The team also assists with other pressing needs, such as completing financial assistance applications for services, applying for health insurance, connecting to transportation services, finding affordable housing, and much more. 

Get In Touch With Us

Patients can get in touch with the Lamoille Community Health Team several ways:

  1. Your PCP introduces you to the Community Health Team when a need is identified during a primary care visit. This introduction happens face-to-face during your doctor’s appointment.
  2. Your PCP makes a referral to the Community Health Team. You will receive a call very shortly after the referral is made.
  3. You ask your PCP for a referral to the Community Health Team for one or more services that they provide.
  4. You or a family member contacts the Lamoille Community Health Team directly by calling 802.888.6017.
