Pediatric Behavioral Health Care Coordinator

As a member of the Community Health Team and Behavioral Health & Wellness, the Pediatric Behavioral Health Care Coordinator works with pediatric patients and their families/ caregivers to identify their health goals. Collaboratively create with their practitioners a care plan that will support them in achieving their goals. Responsible for providing care coordination, transitional care interventions, community resource referrals, behavioral health brief intervention and counseling as well as mental health and substance misuse screening and assessment. This position will aim to develop a therapeutic relationship with patients, while assisting them through the process of better health by providing counseling support, encouragement, and education. Must understand family systems, community resources, insurance, regulatory and legal system issues relevant to improved health outcomes for patients. This position works as an integral part of the pediatric primary care team providing direct service within patient exam rooms and scheduled office visits.

Educational / licensure requirements: Clinical mental health master’s degree, or related discipline appropriate to this position. Preferred –  current, active Vermont LICSW, LPM or LCMHC license. Three years of specialized experience in or related to this field preferred.
