The MAT Clinical Care Coordinator is responsible for coordinating and providing the therapeutic care and treatment of patients receiving Medication Assisted Therapy (MAT) for opioid addiction and is located in offices of physicians prescribing Buprenorphine.

The Medication Assisted Treatment (MAT) Team is a dynamic group of Licensed Alcohol and Drug Counselors and Registered Nurses working with providers across the Lamoille Valley area who are currently providing opioid replacement therapy and comprehensive care coordination.
As a Clinical Care Coordinator this role works collaboratively with patients and practitioners to provide substance use and mental health screening, create supportive care plans, referrals to community resources and brief intervention. MAT Care Coordinators specialize in opioid use but also provide support and resources for any substance use or addiction related challenges. They will also work closely with practitioners prescribing Suboxone or buprenorphine to provide comprehensive care management to current patients, including individual or group counseling.
This position will work closely with the primary care office teams at both Lamoille Health Family Medicine, Morrisville and Stowe.
KNOWLEDGE AND EXPERIENCE:
Requires active Vermont Licensed Drug & Alcohol Counselor license. Strong social work skills and experience in medication assisted treatment is preferred. Working toward your licensure? We still want to hear from you and see if this role could be a good fit to meet your licensure goals!
